All treatment costs must be paid in full at each visit to keep plan in effect. All covered services must occur within the year of enrollment and cannot be carried over to the next year. Hospital services are not covered. Effective date is from the day you enroll and your membership remains in effect for 1 year. At the end of each year of enrollment you will have the option to renew. Membership is nontransferable to another party or family member and cannot be used in conjunction with another insurance. Membership is nonrefundable and subject to change annually.
